Marlies Talk 15/16 - Is Sheldon worth Keefe'n around?

On a side note, if Arcobello is playing so well why not call him up? There's still time for him to play his way to a 3rd or 4th round pick. Or is he pretty much toast in terms of a NHL career?

He's pretty much always been an exceptional offensive producer at the AHL level, and he's largely a known quality at the NHL level, too. With a month left before the deadline, there's really not enough time for him to increase his value much, if any.

Last year around the deadline two players got dealt for third round picks(essentially). Those players were Sean Bergenheim and Jiri Tlusty and, at the time of being dealt, Bergenheim had 18 points in 39 games and Tlusty had 13 goals and 23 points in 52 games. Both were/are pretty legitimate NHL forwards.

It seems pretty unlikely that even if you brought up Arcobello and he played well that he could turn into that kind of player.

He had 0 points in 11 games with the Leafs this season.

Hell of an AHLer though.
 
Keefe confirms that Nylander will return to the line-up tonight. He's been practicing on a line with Brown and Soshnikov recently.
 
It also sounds like Nylander is still set to play in the AHL All-Star game, happening this Sunday and Monday. Leivo, Brennan, and coach Sheldon Keefe will also be representing the Marlies there.
 
I feel like that took a very long time. Then again, I'd rather be safe than sorry. I hope he hasn't lost too much from getting his bell rung like that.

Man the more I think of that hit the angrier I get.

A month feels like pretty standard for a not super serious concussion, actually. If it was much longer, then I'd be really concerned.

Nylander revealed too that after he resumed skating he was sidelined again with appendicitis.
 
Hopefully our resident doctor can chime in and explain this a little bit. I googled appendicitis and it generally brought me to stuff about appendectomies. But it doesn't sound like Nylander had surgery for this, so what exactly would the difference be?

Sometimes it can be treated with antibiotics.

From wikipedia on it:

Acute appendicitis is typically managed by surgery. However, in uncomplicated cases antibiotics are both effective and safe. While antibiotics are effective for treating uncomplicated appendicitis, 20% of people had a recurrence within a year and required eventual appendectomy.

The standard of care for appendicitis remains appendectomy, most often laproscopically.  There is a growing trend to treat appendicitis conservatively with antibiotics (Ciprofloxacin + Metronidazole) but the appropriate population really hasn't been established for who is the best group to do this in.  There is anywhere from a 20-40% rate of reoccurrence of appendicitis in those treated conservatively.  It also doesn't guarantee that the appendicitis won't get worse and could become a perforation with abscess collection often resulting in a longer recovery period and potential conversion to an open surgery.

The interesting thing is that up to 7-15% of all appendectomies are done and often find a somewhat normal appendix.  That number is decreasing with the use of medical imaging but it is interesting because traditionally appendicitis is more of a clinical diagnosis and not really a diagnosis meant to be made by Ultrasound/CT.

In general if you are looking to treat someone with antibiotics instead of with surgery you are looking at someone with a murkier picture.  Their pain doesn't entirely fit with true appendicitis (Peri-umbilical pain that gradually radiates to the right lower quadrant of the abdomen over a 12-24 hour period...pain that gets progressively worse and doesn't wax and wane...eventual development of nausea and anorexia...eventual progression to a surgical abdomen (rigid, severely tender, showing peritonitic signs)). 
The patients that still appear mostly well, maybe have a low-grade fever and a minimally elevated white blood cell count, and are really only minimally tender at McBurney's point seem to do well with antibiotics alone but it is not a guarantee.

For someone like Nylander I'm sure that the logic was that he was overall doing ok, that surgery would have set him back further in his recovery and he didn't want to risk being out for another month.
 
I had appendicitis at six years old and can still remember the pain. It felt like a hot knife stabbing me right in my lower right abdomen. My twin brother had a burst appendix when he was four and almost died. He was in hospital for a month. Thankfully my parents knew the signs and got me to the hospital right away. I was out in a week at most.

They have an 80% win percentage right now, no other team is at 70%.

They also have a better goal differential than the 2nd and 3rd placed teams combined.

There'll be a ton of pressure to win it all this year. I really hope they are up for it!
